Milwaukee Montessori School Children's House Half-Day Enrollment Contract

2013-2014 School Year

This Contract is entered into by and between Wisconsin Montessori Society, Inc., d/b/a Milwaukee Montessori School, 345 N. 95th Street, Milwaukee, WI 53226 (“School”) and the parent(s) or guardian(s) whose signature appears below (“Parent”).

 

In consideration of the mutual promises set forth herein, together with other good and valuable consideration, the receipt and adequacy of which are hereby acknowledged, the parties hereto accept the following terms and conditions governing enrollment, attendance and obligations at the School:

 

1. Enrollment: Pursuant to the terms and conditions set forth herein, the Parent agrees to enroll and the School agrees to accept the Student into the Milwaukee Montessori School for the 2013-2014 school year (“School Year”), which shall be from Wednesday, 8/21/13 through Wednesday, 6/11/14 for the Academic Program or Monday, 8/19/13 – Friday, 8/15/14 for the Toddler Program, subject to the terms and conditions set forth in this Contract including the excluded days provision contained in this Contract. The term “Academic Program” shall include K3 EA – Early Admission to Half Day Children’s House, K3 – Half Day Children’s House, K4 HD – Half Day Children’s House, K4 FD – Full Day Children’s House, K5 – Full Day Children’s House, LE1 – Lower Elementary, LE2 – Lower Elementary, LE3 – Lower Elementary, UE1 – Upper Elementary, UE2 – Upper Elementary, UE3 – Upper Elementary, JH1 – Junior High, and JH2 – Junior High. The term “Toddler Program” shall include FS HD - First Steps Half Day, FS FD - First Steps Full Day, SS HD - Stepping Stones Half Day, and SS FD - Stepping Stones Full Day. The School reserves the right to issue enrollment for a specific grade. Contracts will not be accepted if parents change the grade.

 

2. Obligations and Responsibilities: The Parent and the Student agree to adhere to the policies, rules and regulations of the School as have been established by the School administration or as may be hereafter promulgated, amended or modified. The Student’s continuing enrollment in the School is contingent upon Parent and Student meeting the School’s standards of acceptable performance and good citizenship and the Student’s satisfactory progress in the program in which the Student is placed. The parties acknowledge that the Student will not be permitted to enter classes or participate in School activities or commencement ceremonies unless the Parent and Student are in full compliance with all obligations of this Contract including but not limited to payment in full of all financial obligations. Contracts and Tuition Deposits will not be offered or accepted for families with past due balances for the 2012-2013 or prior school years. Parent acknowledges having received, read and understands the MMS Family Handbook. The MMS Family Handbook is also available on the school web site.

 

3. Tuition and Fees: 

Academic Program:  If the Student is enrolled in an academic program, the Parent agrees to pay the tuition and all fees as herein after provided for the entire School Year if the Student is enrolled on or before 8/30/13. If the Student is admitted and enrolled after 9/30/13, the annual tuition will be prorated for the School Year based on the remaining number of months in the School Year. The Processing Fee on Payment Plans is not prorated or refunded. Parents will be charged and agree to pay the total cost of a full month’s tuition for any month in which the child is enrolled at the School regardless of the number of days the student is enrolled or attends in any given month.

 

Toddler Program: If the Student is enrolled in a toddler program, the Parent agrees to pay the tuition and all fees as herein after provided for the entire School Year if the Student is enrolled on or before 8/23/13. If Student is admitted and enrolled after 8/23/13, the annual tuition will be prorated for the School Year based on the remaining number of weeks in the School Year. The Processing Fee on Payment Plans is not prorated or refunded. Parents will be charged and agree to pay the total cost of a full week’s tuition for any week in which the child is enrolled at the School regardless of the number of days the student is enrolled or attends in any given week. Tuition and fees will be prorated for Students advancing from Toddler Program to Children’s House (with or without childcare) mid-year.

 

Applicable to all programs, if, after completing the Enrollment Contract, the Parent notifies the School in writing by July 1, 2013, that the Student will not enroll for the School Year, this Contract shall be cancelled and neither the Parent nor the School shall have any further liability or responsibility under this Contract. The Processing fee and Enrollment Fee of $500.00 paid herewith to the School shall be retained by the School in the event of such cancellation and is non-refundable and non-transferable. This enrollment fee is credited towards the 2013-2014 tuition. A $25 service charge will be assessed for program changes that result in decreased program use made to this contract after it has been signed and returned. School initiated changes (i.e., Half to Extended Day Children’s House, Toddler to Children’s House or mid-year graduates) and changes that result in increased program use are exempt from this charge. Tuition and fees, other than the Enrollment Fee of $500 and the Processing Fee, will be forgiven or refunded if the student withdraws in writing from the School as follows:

Toddler ProgramMONTH - PERCENTAGE FORGIVEN/REFUNDED July, 2013 - 91%; August, 2013 - 82%; September, 2013 - 73%; October, 2013 - 64%; November, 2013 - 55%; December, 2013 - 46%; January, 2014 - 37%; February, 2014 - 28%; March, 2014 - 19%; April, 2014 - 10%; May, 2014 - 5%; After May 31, 2014 - 0%.

 

Applicable to all programs, Parent acknowledges and agrees that parent will be responsible for any and all costs incurred by the School in collecting any tuition and fees due and owing to the School pursuant to this Contract.

 

4. Contract Conditions: 

Completion of this Enrollment Contract does not create a binding contract with the School. This Contract is not binding on the School until the $500 non-refundable enrollment fee is received by the School and the parent(s) receives email confirmation by an authorized representative of the School.

The School reserves the right to decline admission to any student and does not guarantee placement of any student in any class or activity. The term of this Contract shall be for the 2013-2014 School Year only, and no payments or other obligations hereunder shall be transferable to any other student or term.

 

Academic Program: This Contract is further conditioned upon Student’s completion of the current school year, if currently enrolled, in good standing. Should the Student fail to complete the current year’s work successfully, the School at its option may terminate this Contract or recommend a different program placement within the School. If parent does not consent to this alternative placement, this Contract shall be cancelled. The Enrollment Fee of $500.00 and the Processing Fee paid herewith to the School shall be retained by the School in the event of such cancellation and is non-refundable and non-transferable.

 

Toddler Program: This Contract is further conditioned upon Student’s making satisfactory progress in the Program in which the Student is placed in the School. If the School determines, in its sole discretion, that the Student is not making satisfactory progress in the Program at any time during the effective dates of this Contract, the School may terminate this Contract. In lieu of terminating the Contract of a Student who is not making satisfactory progress, the School may at its option offer a different program placement within the School. If Parent does not consent to the alternative placement offered by the School, this Contract shall be cancelled. The Enrollment Fee of $500.00 and the Processing Fee paid herewith to School shall be retained by School in the event of such cancellation and is non-refundable and non-transferable.

 

5. Contracted Childcare – Academic Programs only: Childcare fees are based on the academic School Year. The childcare program is open only to students who attend one of Milwaukee Montessori School’s Academic Programs. Childcare for School Preparation, Winter Recess and Spring Break is not included in these fees but is available at an additional cost. During regularly scheduled professional days and other days that School is open, but Montessori classes are not in session, children who are enrolled in childcare five days a week will have full coverage. Children enrolled for fewer than five days will only be covered to the extent of this Contract. For example, if a child enrolled for four childcare days needs coverage during the fifth day when Montessori classes are not in session, Parent must pay an additional fee for care during that day. The same is true for children enrolled for fewer days. The Childcare Program will not be in session and no services will be provided on the following days: 9/2/13, 11/28/13, 11/29/13, 12/24/13-12/26/13, 12/31/13, 1/1/14, 1/20/14, 4/18/14, and 5/26/14.

 

6. Excluded Days – Academic and Toddler Programs: 

The Academic Program will not be in session and no services will be provided on the following days: 9/2/13, 10/4/13 early dismissal at 11:30 a.m., 10/10/13, 10/11/13, 10/14/13, 11/8/13 early dismissal at 11:30 a.m., 11/11/13, 11/27/13-11/29/13, 12/19/13 early dismissal at 11:30 a.m., 12/20/13-1/1/14, 1/20/14, 2/7/14 early dismissal at 11:30 a.m., 2/10/14-2/14/14, 3/13/14, 3/14/14, 3/17/14, 4/18/14-4/25/14, 5/23/14, 5/26/14 and 6/11/14 early dismissal at 11:30 a.m.. In addition to these dates, the School may, in its sole discretion, close the Academic Program on additional days when the School determines, in its sole discretion, that the health, welfare or safety of the Students or Faculty require such a closure. In the event of such a closure, there will not be any refund of tuition and/or childcare costs.

 

The Toddler Program will not be in session and no services will be provided on the following days: 9/2/13, 11/28/13, 11/29/13, 12/24/13-12/26/13, 12/31/13, 1/1/14, 1/20/14, 4/18/14, 5/26/14, 6/12/14, 6/13/14, 7/4/14 and 8/4/14-8/8/14. In addition, the School may close the Toddler Program on additional days when in its sole discretion, the health, welfare or safety of the Students or Faculty require such a closure. In the event of such a closure, there will not be any refund of tuition and/or child care costs.

 

7. Tuition Payment: At any time that Parent is in default under the terms of this Agreement, the unpaid balance will become immediately due and payable if the default is not cured within fifteen (15) calendar days of written notice to Parent. Parent understands there will be a late fee of $29.00 for any payment more than five (5) days late and a charge of $29.00 for any checks returned for insufficient funds. In addition, Parent acknowledges and agrees that Parent will be responsible for any and all costs and fees incurred by the School in collecting any amount due and owing to the School under this Payment Plan. There shall be no penalty to Parent for early payment of tuition.

 

Parents must select one of the following payment options and by doing so agrees to comply and abide by all terms of this Agreement. Applicable to all programs, tuition and fees shall be paid directly to the School’s Billing Agent and the Enrollment Contract must be completed by the contract due date. Payment Due Date selection to be chosen with the Billing Agent.

 

A. Full Tuition Payment Plan: Tuition and fees shall be paid in full on or before July 5th or 10th, 2013, as selected with the Billing Agent. Parent will be provided with the full terms, payment procedures, and other information regarding the Full Tuition Payment Option prior to the first payment being due.

 
B.  50/50 Payment Plan:
 

(1) Academic Program: Tuition and fees, including a $25 processing fee, shall be paid in installments of 50% of the annual tuition and fees on or before July 5th or 10th, 2013 with the balance of 50% on or before January 5th or January 10th, 2014, as selected with the Billing Agent. Parent will be provided with the full terms, payment procedures, and other information regarding the 50/50 Payment Option prior to the first payment being due.

 

(2) Toddler Program: Tuition and fees, including a $45 processing fee, shall be paid in installments of 50% of the annual tuition and fees on or before July 5th or 10th, 2013 with the balance of 50% on or before January 5th or January 10th, 2014, as selected with the Billing Agent. Parent will be provided with the full terms, payment procedures, and other information regarding the 50/50 Payment Option prior to the first payment being due.

 
C. Monthly Payment Plan:
 

(1) Academic Program: Tuition and fees, including a $45 processing fee, shall be paid in ten (10) monthly installments, each in the amount identified as the Total Monthly Payment, beginning July, 2013, and due on the 5th or 10th day of each consecutive month, as selected with the Billing Agent, until full payment is made. Parent will be provided with the full terms, payment procedures, and other information regarding the Monthly Payment Option prior to the first payment being due.

 

(2) Toddler Program: Tuition and fees, including a $90 processing fee, shall be paid in twelve (12) monthly installments, each in the amount identified as the Total Monthly Payment, beginning July, 2013, and due on the 5th or 10th day of each consecutive month, as selected with the Billing Agent, until full payment is made. Parent will be provided with the full terms, payment procedures, and other information regarding the Monthly Payment Option prior to the first payment being due.
